PLEASE READ THE FOLLOWING IMPORTANT INFORMATION BEFORE PROCEEDING:

• You must select at least one adoptable dog of interest from the designated drop-down screen when completing this application.

• Please be advised that an application approval does NOT guarantee you the dog you are applying for as the dog may already have several applications submitted.

• Please read each question carefully and be sure to answer thoroughly.  Submitting an incomplete application will only delay the application processing time.

• Please complete only one application, regardless of the number of dogs you are willing to adopt from our organization.  If you are approved, your approval is accepted for all of our dogs.

• Applications are good for six months! After six months you will need to fill out a new application. 

• If the dog you applied for is no longer available or not a good fit, we are happy to work with you to find a dog that best suits you and your lifestyle.

• Once your application is approved, an adoption specialist will contact you with the status of the dog(s) you applied for.

ELEVENTH HOUR RESCUE RESERVES THE RIGHT TO DENY ANY APPLICATION BASED ON OUR STANDARDS AND POLICIES, AND THE REASON FOR THE DETERMINATION IS CONFIDENTIAL.
